Make a Claim
We are always trying to make things easier for our members and
claiming is just one area we have focused on improving for you. There
are a variety of ways to make a claim at HIF so choose the one that is
most convenient for you.
Register for direct credit: Once
your
claim is received and approved,
we can credit your rebate directly into your nominated bank account if
you wish. Just complete the section on 'direct credit' on the
claim form to register.
How to Make an Ancillary Claim

Claim
instantly: HIF members
can now claim for ancillary services on the spot. Simply swipe your member card through the e-terminal* at the point of service and wait for your claim to be authorised. Your benefits will be deducted from the provider fee automatically , so all you need to
do then is pay any additional out-of-pocket expenses (if any) - easy!
* Available at all HIF Approved Providers that use HICAPS or iSoft e-terminals.
Fast-Track your claim via fax or email:
To Fast-Track* your claim, just download and complete a
claim
form then fax it, along with your itemised
accounts and receipts, to: (08) 9328 1685. Alternatively, scan your claim form and accounts and email it to us at:
claims@hif.com.au * Please note that all Fast-Track claims cannot exceed $700.00 per day. This
means that we will assess:
(a) A single ancillary claim where the provider’s
fee does not exceed a total charge of $700.00*; or
(b) Multiple
ancillary claims that total no greater than $700.00.
Claim by mail:
To submit your claim by mail, please download and complete a
claim
form, attach the relevant accounts and then post it to us at:

Claim in person:
If you wish to make a cash claim (up to a maximum of $500), we have six
agents located around Western Australia that you can visit. These are:

How to Make a Hospital or Medical Claim
Hospital: When admitted to a hospital
as a private patient you will be asked to pay the excess if
applicable. After this payment all hospital accounts will be directly
forwarded to HIF on your behalf. Should you receive a hospital account,
please post it directly to HIF.
Medical:
Doctor's accounts may include your surgeon, specialist,
anaesthetist and assistant surgeon.
Please note:- Please ensure
you send all doctors accounts to
HIF not
Medicare (claims sent to Medicare first are not eligible for any
Access Gap payments). Accounts are to be sent in with claim
forms for both Medicare and HIF.
- Medical claims can be sent
by post or presented in person at our head office, though please be
advised that we can not provide immediate payment for AccessGap accounts
presented in person.
- Inpatient pathology and radiology accounts
can either be submitted to HIF or Medicare first. A claim
form for both HIF and Medicare
are required when submitting these
accounts.
Important
Information
The following information must be provided when
lodging a claim:
- Member's full name, Patient's name, Provider's
name, an itemised account (original copy) and receipt if paid.
- The
member must sign the claim form.
- Claims received without these
details will be returned unprocessed.
- Claims MUST be lodged
within two years of the date of service.
- Claims for services
older than two years will not be processed.
- All documents
relating to a claim will be retained by HIF.
