Answer YES to the question "Are you switching funds" and enter the name of your previous health fund and your member number there.

 

   

 

 

Upon completing your online application, check your inbox for your confirmation email and print the Switching Funds Form attached. (You can also download a Switching Funds form here).

 

Complete the Switching Funds form and sign it, then return it to us by either:

  • scanning and emailing it to info@hif.com.au
  • faxing a copy to (08) 9328 3345; or
  • posting it to us at: HIF, Reply Paid 2221, Perth WA 6847.

Once we have received the form from you, HIF are then authorised to contact your previous health fund and arrange everything on your behalf; including acquiring a Clearance Certificate, cancelling your membership and transferring your full length of cover to HIF.

Important things to remember...

  • If you transfer to HIF on an equivalent or lower level of hospital and/or ancillary cover, you won't need to re-serve any waiting periods already served with your previous fund for those services for which you were already covered.

  • If you are switching to a higher than equivalent level of ancillary cover, we will waive all 2 month waiting periods. That means you can claim straight away on popular ancillary services like dental, optical, chiro and physio.

  • To ensure that all your existing benefits and previously served waiting periods are recognised by HIF, you must switch within 2 months of leaving your previous health fund.

  • While HIF can take care of all the paperwork associated with switching, you are required to contact your previous fund directly to cancel any direct debits that may be setup with them.
