HIF Forms Library

Membership Variation Form - change your level of cover, or add/remove a person from your policy
(please note that is it important to read our latest health insurance brochure to ensure you are aware of any new waiting periods, exclusions, restrictions or waiting periods that may apply to your updated policy)
Switching Funds Form - switch from your current health fund
Rebate Application Form - apply for the Australian Government Rebate on Private Health Insurance
Rebate Tier Update Form - advise HIF of the salary tier that will apply to your policy on July 1 (members only)
Direct Debit Application Form - save time and pay by Direct Debit *
Student Declaration Form - advise us if a dependant is (or isn't) eligible to remain on your policy
Change Primary Member Form - put someone else in charge of your policy
Spouse/Agent Authority Form - nominate another person to access your details and claim on your behalf.
Payroll Deduction Form - apply for (or cancel) payroll deduction **
Membership Suspension Form - leaving Australia for between 2 - 24 months? Suspend your policy ***
Reinstatement After Suspension Form - just returned from a long trip away? Reinstate your membership
HIF Member Claim Form - lodge a claim for medical and/or ancillary services
Medicare Claim Form - lodge a claim directly to Medicare
Two-Way Medicare Claim For m - please only use this form if you have been advised to do so by HIF
